The best alternatives to dermal fillers: what you need to know

Posted on 27th June 2025 by Dr Rekha Tailor

Dermal fillers have transformed the landscape of non-surgical aesthetic treatments. Known for their ability to restore lost volume, lift the face, and smooth ageing skin, fillers are widely praised for their immediate and often transformative results.

However, fillers aren’t for everyone, and some people are exploring the best alternatives to dermal fillers. In this blog, we’ll break down what fillers are, when fillers are not suitable and what other treatments exist, and why there isn’t a direct alternative to fillers.

What are dermal fillers?

As we age, the natural fat pads in the face shrink and descend, collagen production slows, our bones shrink and the skin becomes thinner. This results in less volume and ‘fullness’ in the face, as well as sagging skin. This can lead to concerns such as hollow cheeks, under-eye shadows, and nasolabial folds. These are also commonly seen in ozempic face, where rapid weight loss accelerates facial volume loss and skin laxity.

Dermal fillers are injectable treatments designed to restore lost volume, smooth out lines, and provide lift in areas which have lost fullness. Most commonly made from hyaluronic acid – a naturally occurring substance in the skin – they also add hydration and plumpness to targeted areas. Fillers can be used in areas such as the temples, cheeks, under-eyes, lips, jawline, and nasolabial folds, and they offer immediate, visible results with minimal recovery time.

Why fillers are the gold standard for volume loss

Ultimately, if you’re struggling with moderate or advanced volume loss, fillers are the most effective and predictable solution. They offer a level of immediate correction that other treatments simply can’t replicate.

And when fillers are administered by a doctor with extensive experience, dermal filler results can look incredibly natural. At Health & Aesthetics, we take a holistic approach to fillers, considering the whole structure of the face and uniqueness of each individual, rather than treating areas in isolation. This results in a balanced and authentic-looking result.

When alternatives to fillers make sense

So when might an alternative to dermal fillers be the better choice? It ultimately depends on your skin goals. If you’re experiencing early signs of skin ageing – but not necessarily volume loss yet – then an alternative to dermal fillers may be more preferable and provide fantastic results.

Many aesthetic concerns aren’t about volume at all. Issues like uneven skin tone, fine lines and wrinkles, dullness and pigmentation often require a very different approach.

Some effective and popular alternatives to dermal fillers include:

Polynucleotides – an alternative to under-eye fillers

The skin around the eyes is very delicate and tends to lose collagen and elastin much quicker than other areas. As this starts to thin and sag, it can cause dark circles to appear under the eyes in the early stages of ageing.

Dermal fillers can work well in the under-eye region, but only if there’s volume loss also present. Lots of people with dark circles under the eyes don’t have volume loss and so fillers aren’t the right solution.

Polynucleotides have emerged as a safe alternative to under-eye fillers, especially for those with dark circles. These injectable treatments are derived from DNA fragments that help:

  • Stimulate tissue regeneration
  • Improve skin texture and elasticity
  • Boost hydration

Instead of replacing volume, polynucleotides enhance the overall skin quality, increase hydration and improve skin tone, helping to reduce dark circles. This makes polynucleotides ideal for younger clients or those who want to avoid volume-based treatments in this sensitive area.

JULÄINE of Sweden

JULÄINE of Sweden is a cutting edge treatment that stimulates the skin to produce healthy collagen – the protein responsible for giving skin its structure and support. This can help to boost overall skin health, helping to smooth out lines and early signs of ageing. JULÄINE also gives a subtle volume boost, which is why it can be a great alternative to filler for people who don’t need dermal fillers.

JULÄINE is a regenerative injectable treatment, meaning it uses the body’s own natural processes to stimulate the production of more collagen. In this sense, it’s perfect for people who want a natural alternative to dermal fillers. It won’t target volume loss as well as fillers, but it can provide a slight volume boost in people with mild symptoms of volume loss, facial hollowing or sagging.

What’s more, because JULÄINE stimulates the body to create more collagen, results continue to build over time and can last for up to 2 years.

Combining dermal fillers with other treatments

When multiple skin concerns are at play, the best results often come from combining dermal fillers with other treatments. For example, using fillers to add volume and structure in areas lacking fullness, and pairing with treatments targeting different concerns, such as skin texture and tone, skin laxity, or fine lines and wrinkles.

Should I have dermal fillers?

Not sure what’s right for you? The decision to have dermal fillers – or to explore alternatives to fillers – depends on a few key factors: your specific concerns, your age, your skin type, and your desired results.

If your main issue is loss of facial volume, fillers remain the most effective and direct treatment. They’re safe, long-lasting, and can deliver instant results that restore youthful structure and support. However, if your concerns relate more to another skin issue, such as skin quality, texture, pigmentation, laxity or fine lines then alternatives to dermal fillers or other treatment options may be more suitable and effective.

At Health & Aesthetics, we often see patients who are unsure whether they need volume, skin tightening, a skin refresh or something entirely different – and sometimes, it’s a bit of everything. That’s why we recommend a personalised consultation to assess your skin holistically and build a treatment plan that matches your goals.
