The aestheticssector has been moving steadily away from the era of heavy volume and obvious filler, and towards treatments that work with the skin’s own biology to produce gradual, natural improvement. HarmonyCa and JULÄINE are both part of that shift. Both are injectable biostimulators, both encourage the skin to produce its own collagen, and both have gained significant attention in the UK aesthetic market over the past couple of years.
But they are not the same treatment, and understanding what sets them apart is genuinely useful if you are trying to decide which one is right for you. I offer both at Health & Aesthetics, and I choose between them based on what a patient’s skin actually needs, not on which product happens to be trending. This blog sets out the differences clearly.

What Is HarmonyCa?
HarmoniCa is manufactured by Allergan Aesthetics, part of AbbVie, the company behind Juvederm. It is classified as a hybrid filler rather than a pure biostimulator, because it contains two active ingredients that do two distinct jobs.
The first is crosslinked hyaluronic acid (HA), which provides immediate volumisation, hydration and lift the moment it is injected. The second is calcium hydroxyapatite (CaHA) microspheres, which act as a biostimulator. The CaHA triggers the body’s fibroblasts to produce new collagen and elastin, creating a slower, structural improvement in the skin that continues to develop over weeks and months after treatment.
The result of combining these two components is a treatment that does something most injectables cannot: it delivers both an immediate result and a progressive one. Patients leave looking better on the day and continue to see improvement as the biostimulatory effect builds. Clinical data supports results lasting 12 to 18 months or more.
HarmonyCa is primarily indicated for the mid and lower face: the cheeks, pre-jowl area and jawline. It is administered using a cannula, which reduces trauma to the tissue and makes the treatment more comfortable than needle-based injection in these areas. It contains lidocaine, so discomfort is generally minimal.
Important distinction
HarmonyCa is not reversible in the same way as a pure hyaluronic acid filler. Hyalase can dissolve HA fillers entirely; it cannot address the calcium hydroxyapatite component. The CaHA biodegrades naturally over time, but there is no agent to accelerate that process. This is relevant to how treatment is planned and why HarmoniCa should only be administered by an experienced medical practitioner following a thorough assessment.
What Is JULÄINE?
JULÄINE is manufactured by Nordberg Medical, a Swedish biotech company founded in 2017. It is a pure collagen biostimulator based on poly-L-lactic acid (PLLA), a biocompatible and biodegradable material that has been used safely in medical applications for over 50 years, from absorbable sutures to bone fixation pins.
Unlike HarmonyCa, JULÄINE contains no hyaluronic acid. There is no immediate volumising effect and no instant visual improvement on the day of treatment. What it does instead is stimulate a progressive, biological rebuilding of the skin’s collagen framework over time. The PLLA microspheres are injected into the deeper dermal layers, where they activate fibroblasts through a process called mechanical signalling, prompting the skin to produce new Type I collagen at its own natural rhythm.
What distinguishes JULÄINE from older PLLA-based treatments such as Sculptra is the technology behind its microsphere production. JULÄINE uses proprietary LaSynPro technology to produce highly uniform PLLA microspheres with a controlled size and shape. This matters clinically because irregular or inconsistent microspheres are associated with a higher risk of granuloma (nodule) formation. The uniformity of JULÄINE’s particles contributes to its strong safety profile and its smooth, even integration into the tissue.
A standard course is three sessions, typically three to four weeks apart. Results build progressively and can last well beyond 18 months, because the improvement is based on genuine collagen regeneration rather than a product sitting in the tissue.
Managing expectations
JULÄINE is not the right treatment for patients who want to see a significant change on the day. If immediate improvement is important to you, HarmoniCa or traditional dermal fillers are likely to be more appropriate. JULÄINE is for patients who understand and embrace a gradual process, and who want results that develop naturally over months rather than days. In my experience, the patients who get the most out of JULÄINE are those who are prepared to wait for it.
The Key Differences
Mechanism of action
This is the most fundamental distinction. HarmonyCa works through two mechanisms simultaneously: immediate volumisation from the HA and progressive biostimulation from the CaHA. JULÄINE works through a single mechanism only: pure collagen stimulation via PLLA microspheres. Neither is superior in the abstract; they are simply suited to different patient needs.
Immediacy of results
HarmonyCa produces a visible change on the day of treatment. The HA component provides immediate lift and volume, and whilst there will be some settling over the following weeks, the before-and-after is apparent immediately. JULÄINE produces no meaningful change on the day. The first signs of improvement typically become visible after four to six weeks, with the full effect developing over three to six months. Some patients find this liberating; others find it difficult. Knowing which camp you fall into matters.
Where each one is used
HarmonyCa is designed for the mid and lower face, particularly the cheeks and jawline. It is not used in the lips, under the eyes, or in areas requiring very superficial injection. JULÄINE has a broader range of indications and can be used across the face, including for nasolabial folds, marionette lines, the temples and areas where diffuse laxity needs addressing. It can also be used in the neck and décolleté in some protocols.
Number of sessions
HarmonyCa is typically a single-session treatment, with a review at four to six weeks and maintenance every 12 to 18 months. JULÄINE requires a course of three sessions, spaced three to four weeks apart. This is worth factoring into both your time commitment and your overall treatment budget.
Longevity
Both treatments offer longevity that exceeds traditional HA fillers. HarmonyCa’s clinical evidence supports results of 12 to 18 months. JULÄINE, because it is based purely on regenerated collagen, tends to have an even longer duration, with many patients retaining meaningful improvement well beyond 18 months. This is consistent with the broader evidence base for PLLA biostimulators, which are known for their lasting results.
Reversibility
Pure hyaluronic acid fillers can be dissolved with Hyalase, which makes them reversible if results are not what the patient hoped for. Neither HarmoniCa nor JULÄINE is reversible in the same way. The CaHA in HarmonyCa and the PLLA in JULÄINE both biodegrade naturally over time, but neither can be dissolved on demand. This is not a reason to avoid either treatment, but it does reinforce why careful patient selection and thorough consultation are non-negotiable.
So Which One Should You Choose?
In most cases, the answer comes down to three things: what your skin needs, what kind of result you want and how you feel about the time it takes to get there. Here is a practical framework:
- Choose HarmonyCa if you have facial laxity and some volume loss in the mid or lower face, and you want an immediate visible result alongside longer-term biostimulation. It is also a good choice if you are newer to biostimulatory treatments and want to see a clear change on the day.
- Choose JULÄINE if your main goal is progressive, diffuse skin tightening and collagen rebuilding across a broader area of the face. It is particularly well suited to patients with early-to-moderate laxity who want natural, gradual improvement and are not looking for volume enhancement.
- Consider JULÄINE if you have experienced the ‘pillow face’ effect from previous over-treatment with fillers and want a treatment that firms and tightens without adding any volume whatsoever. JULÄINE does not add volume; it simply rebuilds the skin’s own structure.
- Consider a combination approach if you have both volume loss and diffuse laxity. HarmonyCa in the cheeks and jawline alongside JULÄINE for broader facial tightening is a logical and effective pairing. We can plan this properly during your consultation.

My View
I think both HarmonyCa and JULÄINE represent genuinely good additions to what we can offer patients. We are in a period of aesthetic medicine where the emphasis is correctly shifting towards quality and longevity over volume, and both of these treatments reflect that direction.
My honest view is that HarmonyCa has the advantage of versatility: the dual mechanism means it suits a wider range of patients, including those who want something more immediate. JULÄINE has the advantage of purity: if a patient wants nothing but their own collagen to do the work, with no added volume at all, PLLA biostimulation is the most direct route to that outcome.
Neither treatment is a substitute for proper consultation, proper assessment and a practitioner who is honest about what a treatment can and cannot achieve. The products are only as good as the decisions made before they are used.
Frequently Asked Questions
Can I have HarmonyCa and JULÄINE at the same time?
Yes, in principle, though the timing and sequencing would depend on your individual treatment plan. In many cases it makes sense to complete one course before beginning the other, so that we can properly assess the result and plan the next step. Occasionally both can be done on the same visit. This is something we would discuss and agree during your consultation.
Are either of these treatments painful?
HarmonyCa contains lidocaine and is typically administered via cannula, which makes it a comfortable treatment for most patients. JULÄINE is injected using fine needles and does not require anaesthesia for most patients. Most people describe both treatments as very tolerable.
What is the downtime?
Both treatments have minimal downtime. You may experience mild redness, swelling or small injection-site marks for 24 to 48 hours. Bruising is possible with either treatment and more likely around certain areas of the face. We recommend avoiding strenuous exercise, alcohol and excessive heat for 24 to 48 hours after treatment.
Is JULÄINE the same as Sculptra?
Both JULÄINE and Sculptra use poly-L-lactic acid as their active ingredient, but they are different products with different formulations, different microsphere technologies and different clinical profiles. JULÄINE uses proprietary LaSynPro technology to produce highly uniform microspheres, which its manufacturer associates with a lower risk of granuloma formation compared to older PLLA products. They are not interchangeable, and clinical experience with one does not automatically translate to expertise with the other.
How soon will I see results from JULÄINE?
Most patients begin to notice early improvement around four to six weeks after the first session. The full effect typically takes three to six months to develop, as it depends on the body’s own collagen production cycle. By the end of the three-session course, improvement is usually clearly visible. This is a treatment that rewards patience, and patients who commit to the full course consistently report the best outcomes.
I am concerned about getting a lumpy result. Is that a risk?
Granuloma formation, where small lumps develop under the skin, is a recognised risk with PLLA-based treatments. JULÄINE’s LaSynPro technology is specifically designed to minimise this risk through the uniformity of its microspheres, and its safety profile in clinical practice is strong. That said, the technique of the practitioner matters enormously. Correct injection depth, appropriate dilution and the right patient selection all contribute to avoiding this outcome. It is one of the reasons why practitioner experience with biostimulators specifically is so important.
